Minimum Qualifications:
BS or Pharm D. degree
Working knowledge of pharmacy operations, processes and policies in a hospital and retail setting
Demonstrates strong communication, organization and planning skills
Licensure, Registration, Certification:
One of the following:
Parenteral licensure/eligibility
Preceptor License/eligibility
Desired Qualifications:
PGY1; PGY2 or Residency completion
Experience in Specialty Pharmacy preferred
Experience in Oncology setting preferred
Basic knowledge of 340B
Desired Licensure, Registration, Certification:
Board Certification Preceptorship
Ensures and provides patient care that optimizes medication therapy and promotes health, wellness and disease prevention through Comprehensive Medication reviews, Disease Specific Medication Reviews and Targeted Interventions to assess appropriateness of all medications, identify barriers to adherence, optimize medication therapy and improve clinical outcomes.
Facilitates the development, implementation and maintenance of complex, in-depth clinical services that promote desirable patient outcomes through the use of safe, cost-effective drug therapies through delivery of MTM (Medication Therapy Management) services.
